罚单显示，浙江温岭农村商业银行股份有限公司的主要违法违规事实（案由）为：贷款管理严重不审慎。
针对上述违法违规行为，国家金融监督管理总局台州监管分局对其罚款45万元；对郑仪禁止从事银行业工作5年；对郭振宇警告。
