潘氏古墓，位于福建省南安市康美镇，俗叫潘厝墓。因墓陵大，为当地一标志，村民于墓陵口附近筑房而居，形成村落，故地名：墓仔口。
据县志载，明·按察史潘洙葬于此。潘洙，明万历十七年(1589年)进士，授卢州教授，升工部都水司主事，后转任吏部主考、选考功郎中、升江西参政、广东左布政使等职。
