Topic:GLP-1 Receptor Agonist–Based Therapies in Cardio-Renal-Metabolic Disease: Clinical Evidence, Mechanisms, Safety, and Unmet Needs

Main Purpose
We are pleased to invite submissions for a special issue focusing on the role of glucagon-like peptide-1 receptor agonist (GLP-1RA)–based therapies in cardio-renal-metabolic diseases.
Originally developed as glucose-lowering agents, GLP-1RAs have subsequently been evaluated for their effects on body weight, cardiovascular outcomes, kidney function, and other metabolic endpoints. Clinical trials and real-world studies have reported favorable outcomes in selected patient populations; however, the magnitude, consistency, mechanisms, generalizability, and long-term durability of these effects remain areas of active investigation. Important questions also remain regarding differences among individual agents, appropriate patient selection, combination treatment strategies, long-term safety, treatment discontinuation, accessibility, and cost-effectiveness.
This special issue aims to provide a balanced and critical assessment of the current clinical and translational evidence on GLP-1RA-based therapies. We welcome original research, reviews, perspectives, and other scholarly contributions addressing beneficial, neutral, or adverse outcomes; underlying biological mechanisms; comparative effectiveness; real-world implementation; combination therapies; safety and tolerability; and remaining evidence gaps.
By bringing together perspectives from cardiology, endocrinology, nephrology, obesity medicine, and related disciplines, this collection seeks to clarify the potential role and limitations of GLP-1RA-based therapies in integrated cardio-renal-metabolic care and to identify priorities for future research and clinical practice.
Outline of Pathways
1. Mechanisms of GLP-1RA-based therapies beyond weight loss and glycemic control, including potential vascular, myocardial, renal, inflammatory, and metabolic effects.
2. Clinical effects and underlying mechanisms of GLP-1RA-based therapies in obesity-related heart failure with preserved ejection fraction.
3. GLP-1RA-based combination therapies, including combinations with sodium–glucose cotransporter 2 inhibitors and other cardiovascular, renal, or metabolic treatments.
4. Effects of GLP-1RAs in metabolism-associated cardiovascular diseases, including atherosclerotic cardiovascular disease, heart failure, and cardiometabolic risk states.
5. Cardiovascular, renal, and metabolic outcome trials of next-generation single, dual, and triple receptor agonists, including GLP-1/GIP and GLP-1/GIP/glucagon agonists.
6. Comparative effectiveness and potential heterogeneity of treatment effects across different agents, populations, and clinical settings.
7. Real-world evidence on effectiveness, adherence, persistence, treatment discontinuation, accessibility, and implementation.
8. Safety, tolerability, contraindications, and risk–benefit considerations in patients with cardiovascular disease, chronic kidney disease, obesity, and diabetes.
9. Long-term treatment durability, weight regain after discontinuation, and the clinical implications of treatment interruption.
10. Cost-effectiveness, health equity, and access to GLP-1RA-based therapies.
11. Methodological challenges and limitations in clinical trials, observational studies, and comparative-effectiveness research.
12. Evidence gaps in older adults, frail patients, individuals without diabetes, and patients with advanced chronic kidney disease or multiple comorbidities.
13. Patient selection, treatment sequencing, and implications for multidisciplinary cardio-renal-metabolic care.
