罚单显示，恒丰银行股份有限公司东营分行的主要违法违规事实（案由）为：内控管理不到位。
针对上述违法违规行为，国家金融监督管理总局东营监管分局对其罚款30万元；对时任该分行行长王刚禁止从事银行业工作15年。
