普眼菩萨为十二圆觉菩萨之一，亦为观音菩萨之异名。因佛经称其慈眼普观一切众生，谓之“普眼”。大悲世尊，愿为此会诸菩萨众，及为末世一切众生演说菩萨修行渐次，云何思维？云何主持？众生未悟，作何方便普令众生开悟？世尊，若彼众生无正方便及正思维，闻如来说此三昧，心生迷闷，即于圆觉不能悟入，愿兴慈悲，为我等辈及末世众生假说方便。
