国际勃起功能问卷表最开始由Rosen等在1997年设计，可评价阴茎勃起功能，1998年Rosen等为了便于临床实际应用又不影响其准确性，将其简化为5个主要问题的国际勃起功能问问卷表(IIEF-5)。
以下为量表，可自评参考。
5-7分为重度ED；8－11分为中度ED；12－21分为轻度ED；大于22为勃起功能正常。如果分数小于或等于21分时，需到正规医院治疗。
