罚单显示，中信银行股份有限公司平顶山分行的违法违规事实为：贷前调查不尽职、贷后管理不到位。
针对以上问题，国家金融监督管理总局平顶山监管分局对其罚款50万元；对时任新华区支行公司业务部经理郭臣飞警告。
